Insurance Information
You are responsible for providing us with your insurance information, including primary and any secondary insurance information. If the information is not provided to our office, or if the information that is provided is incorrect, you will be responsible for the charges until complete information is provided.
While we often verify benefits, we do so as a courtesy to the patient and cannot guarantee that the information your insurance carrier provided to us is correct. Ultimately, you are responsible for contacting your insurance carrier to verify benefits prior to each appointment. Please call our office if you have any questions about using your insurance.
Out-of-network provider
If we are not in-network with your insurance carrier, we can see you as an out-of-network provider. In this situation you are considered "self-pay" and will be responsible for payment to NSO at the time services are rendered.
Under the No Surprises Act, self-pay patients have the right to receive a "good faith estimate" of the the cost of care at least one business day before services are rendered. If you receive a bill that is at least $400 more than the good faith estimate you can dispute the bill. At your request, we will provide you with an invoice for services rendered so that you may seek reimbursement from your insurance carrier if you wish to do so. We do not submit billing to the insurance carrier when we are out-of-network.
We cannot guarantee that your insurance carrier will reimburse you and we will not accept payment from your insurance carrier.
Payment Policy
Due to the increased difficulty obtaining timely payment of outstanding balances, we require that a credit card be placed on file before services are rendered.
If you are using insurance, our office will do our best to obtain benefit information and estimate the amount that you may owe for services rendered. This estimated amount due will be provided to you prior to the day of your appointment and you will be required to pay this estimate on the day of your initial appointment.
If you are not using insurance, you will be provided with an estimate of the cost of evaluation services prior to your appointment. This estimated amount due will be collected at the initial appointment.
While NSO does accept check, Visa, Discover, or MasterCard, all patients are required to have a credit card on file. This card is only charged if there is an additional balance due after evaluation services are rendered.
