Appointments

Due to the substantial block of time reserved for your appointment, we require a minimum of 48 hours’ notice for cancellation of appointments. An appointment can be canceled by calling our office at 541-306-6456.


If an appointment is not cancelled at least 48 hours prior to the scheduled appointment time, the patient may be charged a minimum of $150 up to the full amount of time which was reserved for the appointment at the rate posted in our office.


The fee may be waived for special circumstances upon discussion with your provider.


If a patient cancels two times without adequate prior notice, NSO reserves the right to decline to see the patient and provide referral to another clinic

Emergencies

Our office is not an emergency clinic, and we do not provide after-hours coverage. If you have an emergency, call 911 or seek immediate assistance at urgent care or the emergency room.


If you have an immediate need, or medical concern, please call your primary care provider’s office or your referring provider.

Electronic Communication Policy

In order to maintain clarity regarding the use of electronic modes of communication during your treatment, NSO has prepared the following policy.

Phone Contact with Our Office

We return calls as soon as possible during normal business hours. However, due to our clinic schedule, we are often not immediately available. When we are unavailable, callers can leave a voicemail message.


Safety

For the safety of our patients and staff, weapons are not allowed in our office. Additionally, with the exception of documented service animals, animals of any kind are not allowed in the office.

Collaboration with your referring provide

The findings from your neuropsychological evaluation will be provided to the referring provider after the evaluation process is complete.

Neuropsychological Services of Oregon, LLC (NSO) provides neuropsychological and psychological services to adults. The goal of the clinical staff at NSO is to complete a comprehensive evaluation that may assist with diagnostic clarification and treatment planning.
